Hello....Our HP d145 will initialize, warm up and the display will read the time and date...everything is fine UNTIL you push copy. The black scanner piece/cartridge thingy (that you can see thru the glass) moves about an inch and then you can hear a grinding noise in the right (upper-I think)corner of the printer. Then the display reads: Scanner Failure....Power off and back on. We have done this repeatedly and we get the same ole thing over and over. lubricated scanner bar and cleaned scanner unit and problem continued.
Your best bet is to clean the underside of the scanner's glass... or see what your line voltage is, I found they sometimes are very picky eaters of electricity- which could be due to another slight flaw the d135/145/155 have which relates to how the power connector on the printer is soldered on. (try resoldering the power connector on the printer motherboard if all else fails.)

Re: Re: hp d145 scanner failure by mandle (9/13/05 3:18 PM) reply
We fixed one today that had 2 scanner assemblies replaced in the past year. What we did was remove the scanner glass and clean it. Then there is a white strip on the underside with a tiny HP logo on it. That's what the scanner calibrates to. Make sure you clean that real good. Then clean the mirrors. Do not use anything wet. Paper towel and dry q-tips.
Re: hp d145 scanner failure by BJ (9/16/05 7:27 PM) reply
OK. Have the same problem with a d155xi. When I attempt to fax or scan it says "warming up" and then goes to "Scanner failure. Turn power off then on again."
It gives me something to do I guess but I get board after 5 or 6 hours and remember I needed to fax my document!
Seriously though. Have any of you figured out a fix? My light just stays on the right side of the scanner bed and doesn't get over to the white strip to calibrate so it isn't that. Could the light be stuck and would opening up the glass and giving it a little shove help? Or hurt?
Have had this problem for a couple of months and HP's solutions and online help are worthless!

Normally there is a sensor your scannner uses to find home. An obstruction, bent/out of place flag,or dirty sensor can throw up the error. Figure out the homing sensor your MFP uses and you should be able to trace the problem.
Re: Re: Re: hp d145 scanner failure by Anonymous (9/22/05 3:50 AM) reply
Most of these HP's I sort out by cleaning the underside of the glass, especially where the scanner registers it's home position. Then clean all the mirrors in the scanner unit. They don't always look dirty but if you shine a torch onto them you will more than likely see a blue "fogging" However I had a d145 last week that had ink leak into the scanner cable connector at some stage & it had corroded. I solved that by replacing the scanner ribbon cable. But usually cleaning solves! Good Luck Dazz UK

I bought a d145 from ebay recently as my old one was getting cranky. I set up the ebay 145 and it would almost every time show "scanner failure, turn power on and off" when I had a job to do. I dismantled the top and scrupulously cleaned the underside and reistalled the glass unit. It seemed to work better but I still got the "scanner failure" but less frequently. So, cleaning the underside of the glass did help but not to my satisfaction. I figured it wouldn't hit my pocketbook to go back in and clean the scanner mirrors, a job I thought would be formidible. If I ruined it, what the heck. I took it apart and took out the scanner device unit out, dismantled the unit and cleaned the mirrors that are clipped into it at various places and angles in the unit. As someone said, the mirrors may look clear but when you wipe them with a cloth or lense cleaning paper you see a film has collected. Not that hard. Put it all together and it appears to have no problems but I will wait and see. I ran several scans and copies to see if it worked. Stayed on between jobs and didn't give me that "scanner failure yah yada". I took digitalpics of the unit as there are some tricks in dismantling. Takes a couple of hours if you are not familar with where all the screws are located. Contact me if you need to see some of the insides. My old unit was used very heavily to copy and I liked the larger ink tanks. I don't like the idea of buying a newer hp and making them richer when I have to go back to the store all the time to buy the smaller tanks. When I wrote this, the scanner failure blip did come up again but when not in use. Not too troublesome at least when I am not in the middle of a copy job or scan job or do one job and then finish it and be forced to turn it off for the next job over and over.
HP tech support said it was probably due to the scanner's lightbar failing to pre-heat to its minimum operating temperature, and that if plugging the AC adapter directly into the wall did not solve it, nothing would -- short of depot service (no replacement parts for the scanner assembly).
I have heard of people getting the scanner going again by blowing hot air on the underside of its housing with a hair dryer!

You need to pry the front plate that covers the buttons. Try a flat head screw driver and wiggle it in front and you will see how to pop it off. Then you will see the screws that are hanging the glass plate assembly up. The top will release once you get those screws off that are under the display plate.

Re: Re: Re: hp d145 scanner failure by Anonymous (7/12/07 8:31 AM) reply
I can send you pics and instructions for disassembly. I switched up to Vista and the hp d145 has apparently been way down on their list as the front to back duplexer feature won't work and there are other problems with its functions with Vista. Phasing out the programming for these old reliable beast is one way to buy up to more expensive and smaller print cartridges. It still works with XP and I recently got the scanner failure again. Cleaning the mirrors and underside of the glass plate worked for about a year. It's worth the try to keep these old big buzzards working as long as you can. I found my d145 to be very reliable and you can't beat the big ink tanks and the resolution for pictures is just as good, if not better, the new HP Officejet Pro L7680 my wife hauled in claiming I was spending too much time tinkering around with the d145. If you can get the scanner unit cleaned and functioning without putting the aluminum foil in the aluminum blub holders in then that is the way to go. After a while you can tear the unit down blindfolded. There are a couple of odd twist and turns with a few of the screws here and there. Make sure you don't let the scanner unit communication tape get up and out of its designated place since it will hang up the scanner unit as it slides and really make a grinding sound. I see that someone on ebay is selling the scanner units that have been tested.
If you send me your email I can send the 28 pictures I have and the instructions. Pass the pictures around. I am traveling soon and may not be able respond to request for the pictures.

Re: hp d145 scanner failure by chaplaw49 (11/2/07 7:00 PM) reply
Nope, no charge just trying to reduce HP's outrageous profits they make from their new but smaller ink cartridges. The D145 are reliable, work great and you can't beat their larger color digital photo prints. You will find that Vista will affect features of these old beast, like the duplexer. A cleaning of the underside of the scanner plate glass may be the only thing you need to do. If you go further, it gets really bizarre but it can be done. Taking the scanner mirror assembly out is something a licensed surgeon can do but if you proceed with due diligence and with good tools you can do it. My pictures are not inclusive but they can guide you to where your common sense can take over.

Re: hp d145 scanner failure by Bert (12/16/07 5:47 PM) reply
Clean the underside of the scanner glass giving particular attention to the white strip. Then clean the scanner bulb (gently, its thin glass) and the mirror (in the long slot) with a cotton swab sparingly moistened with Windex or a similar cleaner.
Officejet D145 – Removing the scanner glass
General The Automatic Document Feeder (ADF) will need to be removed before the glass can be removed. You will also need a T10 Torx head driver approximately 6" long. Driver handles that accept removable bits may be too large in diameter for this particular job.
Removing the automatic document Feeder (ADF) 1. Hinge the ADF upward as if you are going to manually scan a document. 2. In the hinge area there is two black pull latches. Pull them up and lift the ADF straight up to remove it. Some side-to-side rocking may be necessary before it will disengage.
Removing the Scanner Glass 1. Remove the four T10 Torx head screws around the perimeter of glass, i.e. two in the ADF hinge area and one on the right and left. 2. Remove the Control Panel overlay. It pries off. 3. Open the printer as if you are going to access the cartridges and disengage the support legs. Two latches at the base of each leg must be pried outward with a slot style screwdriver to disconnect them. The scanner assembly will now "flop" freely, so steady it while removing the four screws on the underside.
Note: The screw in the front is hidden under the piece that you grasp to gain cartridge access. Two latches must be released to remove it. The screw can now be seen using a flashlight but may be obscured by some wires

Hello Chaplaw49. Could you be so kind as to send a copy of the instructions/photos to ktech257@msn.com
I too am getting the famous scanner failure error and HP's response was to toss out the printer and by a new one. I have the di155xi. Needless to say you do not want to hear what I think of HP... They have gone so down in their home printer division it is beyond any thinkable words.
Or if anyone else has phots or docs please feel free to jump in.
I spoke to an HP service center AKA Radio Shack repair center. He told me this is a common problems with the d135, d145, and d155. He said it is a system board issue that controls the scanner function that goes bad.
The entire scanner assembly needs to be replaced. Problem is no parts are available unless you salvage one. He said he had seven units in the shop that are broken with scanner issues. He uses them for other parts. HP GET SMART OR YOU WILL LOSE THE PRINTER MARKET.....
STUPID VERY VERY VERY STUPID!!!! I am livid to say the least.
Could we disconnect the scanner and just use it as a printer??? I cannot outlay money now for a new printer.....

I am having the same problem. This may be a temporary fix in a crunch. We found that if you unplug your printer for about 5 minutes, replug then turn it back on, this will temporarily fix the scanner failure problem and allow you to print/copy. The scanner failure message does come back on after a short while.

Re: hp d145 scanner failure by Bert (6/1/08 5:05 PM) reply
Removing the automatic document Feeder (ADF) 1. Hinge the ADF upward as if you are going to manually scan a document. 2. In the hinge area there is two black pull latches. Pull them up and lift the ADF straight up to remove it. Some side-to-side rocking may be necessary before it will disengage.
Remove the Scanner 1. Lift the scanner as if changing the cartridges. 2. Disengage the hinged supports by prying apart the latches attaching their lower ends with a slot style screwdriver. 3. Rotate the scanner 90 degrees and remove the metal plate at the left rear of the case. The plate is held in place by two T10 Torx head screws. 4. Unplug the four cables interconnecting the electronics board and the scanner. 5. Lift the scanner straight up and it will disengage from it's hinges.
Remove the Case 1. Inside on the left is a gray ribbon cable. Unplug it. 2. Remove the vertical panels on each side of the paper tray. Reach inside and grasp the edge, then pull outward. 3. Remove the rear access door. 4. Remove the two T10 Torx head screws on the top surface. 5. Release the latch behind the panels removed in step 2. Release the finger latch on the underside of each end. Release the latch behind the two vertical surfaces in the rear. The case will now lift off.
Re: hp d145 scanner failure by Fixed it! (6/7/08 8:54 AM) reply
I followed the instructions posted in disassembling the scanner glass, with the exception of finding out why it would not disengage at the front. So, the only thing I could try was lifting the back part of the glass on the scanner open to use canned air to spray around the sensor area, which is connected to the scanner light at the right of the back of the scanner bed. I figured that from the posts on here, there is a strong possibility that the sensor was being blocked by something light that could be easily removed without touching it by using canned air...such as dust, etc.
While spraying, I sprayed the air all along the pole that the scanner light runs along, then focused on blowing out the actual sensor area really good.
Partially put the printer back together enough to test before fully putting it back together, and it worked!
Bottom line...
- First, unplug the A/C Power adapter at the outlet, not the back of the printer first. The, plug back in and test. This appears to not fix most of the time, but worth a shot, since this is the easiest method.
- If unplugging and plugging back in does not fix, try to blow out the sensor area. Do not worry about being able to remove the scanner glass totally. Just get it open enough to lift the assembly open enough to spray out the sensor area, and see if that does it for you.
Good luck!
Re: hp d145 scanner failure by chaplaw49 (6/8/08 9:31 PM) reply
Well, that was short lived. The error came back, so I had to take it completely apart. Gave it a good cleaning on both sides of the scanner glass, along with cleaning all the mirrors, which is the biggest challenge in making sure they clean up good.
Had intermittent problems, but after going back to the mirrors and making sure they were all clean (I missed a couple), it seems to be working fine.
I would say these mirrors are a huge flaw in the engineering of this scanner. There are way too many mirrors, which gives the scanner that many more possible points of failure.
At any rate, good luck everyone.
***Important note***
The most challenging part I had was getting to the last screw that holds the scanner glass in place at the front of the printer. The trick is removing the keypad. After 10 minutes of "investigating", I found that the keypad "hooks" itself in. To "unhook" it, push or slide the keypad to the right. This will allow the keypad to be lifted out of the tray. Once you have this out, you are able to acess two screws holding the tray that the keypad sits in. The bottom of the tray clips on the left and right sides, so once you have the screws out and you unclip it, the tray should come off. Once you have the tray off, you will finally have access to the screw at the front of the scanner glass.
Gotta love it!

Never mind...I think I figured out what you are doing. I am guessing that may there is some type of luminance rating that needs to be met in order to have proper operation. Great! Another repair to look forward to whenever that happens!
Re: Re: hp d145 scanner failure by Anonymous (7/9/08 12:15 AM) reply
I had the same problem with the finest screw but i fixed it wit a flat screwdriver and then you can use another screw - especially the one picture 10 - to fasten it later on.
OK. So, I my fix was only short lived, until my scnner failure issue showed it's ugly head again.
But, now after several weeks of flawless operation, I think I can safely say that it's truly fixed this time.
My newfound solution? Replace the entire scanner module. Easier said than done, but I managed to find one. I actually found two sources. The first source wanted $150 for it. I couldn't justify that price, so I kept looking, and found an ebay seller who had two left at $40/each (including shipping). Woohoo! I almost bought both, but decided to just get one.
If anyone's interested, I'll look up the ebay seller's info...just let me know...or just run a search. The seller is out of the state of Indiana.
Good luck!

OK...sorry for the delay, but here's the ebay seller...
but first READ THIS!!!
When you replace your scanner module, adjust your power save mode time. The default is 12 hours...I think. I adjusted mine to 1 hour (which is the minimum, because that's the best fit for me, and it's probably the best fit for most average users.
The scanner module failure is due to it breaking down from heat of being on for so long after it's used.
